Application for Manners Class included in Adoption Fee

The ARF Adoption fee covers one manners course taken within 120 days of the adoption date

RELEASE/WAIVER OF LIABILITY: The guardian of student dog acknowledges and understands that he/she is allowing said dog to train at ARF at his/her own risk. Tony La Russa’s Animal Rescue Foundation and its owners, officers, directors, employees, agents, contractors, and affiliates shall not be liable, and the guardian expressly releases and waives any claim of liability, for personal/bodily injury or damages, property damage, or any other type of damage including injury to pet, or for any loss of injury to person or property of pet. This waiver/release is in tended to be a complete release of any responsibility for personal injury or damage and/or property loss or damages sustained by guardian. Guardian also expressly agrees to hold harmless, defend and indemnify Tony La Russa’s Animal Rescue Foundation from any and all claims arising out of visits by said dog of guardian including any and all claims by third parties for loss of or injury or damage to person or property and/or loss of or injury or damage to pet.

If, at any time, the trainer judges that a dog or handler is jeopardizing the safety of people or other dogs in the class, the trainer may request that the dog be removed from class.

Fee: this session is included in the ARF Adoption fee. I understand that I will not receive a refund if my dog does not attend all classes in the session once my registration is confirmed.

Requirements

All adult dogs must arrive with proof of parvo/distemper and rabies vaccination within one (1) year (three (3) year rabies is acceptable).
Puppies must have proof of a series of three (3) DHPP vaccinations (the final one being at 14 weeks or older).
ARF recommends bordatella for all dogs.
