Before beginning the online registration process, please collect the following information.
- Child's date of birth
- Child's current grade level and school name
- Physician and dentist's names and phone numbers (in case of emergency)
- Emergency contact name(s) and phone number(s), someone other than parent/guardian
- List of allergies, current medications and medical conditions/limitations of which ARF staff should be aware
- Credit card information for online payment. (Alternate payment options are available, but may delay registration.)
