Application for ARF Adopters Manners Class

Please complete and submit the form below. You will be contacted to confirm you acceptance into the class for which you are applying.

Requirements

• All adult dogs must arrive with proof of parvo/distemper and rabies vaccination within one (1) year (three (3) year rabies is acceptable).
• Puppies must have proof of a series of three (3) DHPP vaccinations (the final one being at 14 weeks or older).
• ARF recommends bordatella for all dogs.
• Please see class description for any additional requirements or prerequisites.
